Instructions

There are two ways to complete the Human Rights Complaint Form.  One is by filling out the form below and the other is by downloading and copying the form, filling it out by hand, and then sending it to:

Seaside, CA 93955

For both ways, please include copies of filed complaints and right to sue letters.  If you have copies of those documents, they will have to be mailed to the P.O. Box address as there isn’t a way to attach files to the electronic submission.
